What is Information and Assistance?

Provides information and assistance on services and benefits in Winnebago County with an emphasis on individuals with limited family support or have physical or mental impairments. All of Lifescape's caseworkers are also certified SHIP (State Health Insurance Program) counselors.

  • Family Caregiver and Grandparents Raising Grandchildren
  • Supplemental Nutrition Assistance Program (SNAP)
  • Gap Filling applications
  • Homestead Exemption Freeze
  • Housing Applications
  • Low Income Home Energy Assistance Program (LIHEAP)
  • Medicaid applications, redeterminations
  • Prevention of Spousal Impoverishment
  • Qualified Medicare Beneficiary (QMB) applications
  • Specified Low Income Medicare Beneficiary (SLMB) applications
  • Transportation
  • Medical D enrollment, information and temporary funding
