Health & Wellness

At Lifescape Community Services, our health and wellness workshops strive to
reach the health (referring closely to immediate physical or mental state)
and wellness (taking into account general lifestyle and level of fitness) of
the older adults we serve. Through our health and wellness programs,
Lifescape aims to help individuals achieve a higher level of both.

Our Chronic Disease Self-Management Program (CDSMP) and Diabetes Self-Management Program (DSMP), Take Charge of Your Health: Live Well, Be Well workshops are Stanford University based 6 week courses where participants have discussion, develop action plans, problem solve and learn to better manage and live well with chronic conditions.
